A single trip is defined as the day the traveller depart from Singapore and eventually returns back to Singapore.
Annual plan covers you for all travel taken within a 12 month period. There's no limit to the number of trips you can take so long as each trip is no more than a specific days cover by the policy.

Single Trip plans - The trip must not exceed 182 consecutive days
Annual Trip plans - Each trip must not exceed 90 consecutive days
AXA SmartTravel
